Enhanced Care Management

Enhanced Care Management (ECM) is a free, voluntary program for Medi-Cal members who need extra support navigating health care and community services. ECM helps children, youth, young adults, and families with complex medical, mental health, or social needs by connecting them to a dedicated Care Manager who works alongside them in the community. The Care Manager helps coordinate doctor visits, behavioral health services, housing and food resources, and other supports—making it easier to get and stay connected to care. ECM is designed for families facing challenges such as homelessness, serious health or mental health needs, involvement in foster care or the justice system, or transitions from the hospital, and provides personalized, culturally responsive support focused on stability, wellness, and long-term success.
